NW Indiana Membership Enrollment Form

Complete and submit this membership form to become a participating member in the Northwest Indiana Chapter of the National Hook-Up of Black Women, Inc. We are looking for leaders who fully understand the scope of commitment and dedication required to serve (minister to) the needs of others. Before you complete this form and pay the annual membership dues, we encourage you to revisit the membership page of this website. Membership dues are non-refundable if not cancelled within 3 business days after receipt of your check or money order by the NW Indiana Chapter. NHBW is a tax exempt 501(c)3 organization. All members are assigned to committee work. Select at least two committee preferences from the list below. You can serve on both. However, we will ask you to serve on the committee selected where the need is greatest.
